Forms
Electronic forms are available for use by the Deans, Vice Chancellors, and
authorized faculty and staff. Electronic forms may be accessed from the ASU
Home Page under Faculty/Staff Resources. The Computing and Technology section
lists the Electronic Forms page. The left part of the screen lists the Vice
Chancellor's areas, press the Office of the V Chancellor for Business Affairs;
the electronic forms for the Budget Office are listed here. (1) Budget Adjustment-
(utilized by Deans or Vice Chancellors to transfer funds from their accounts
to those departments reporting to them). The Budget Revision Form can be
completed on the screen; however, the form must be printed and routed for
the appropriate approvals and submitted to the Budget Office.
