15-PASSENGER VAN DRIVER APPROVAL PROCESS
1. Applicants must obtain a 15-Passenger Van Driver Approval Form from the sponsoring department's supervisor. Departmental Van Use and Motor Pool Van Use forms are available on the Safety & WC Office website (www.safety.appstate.edu) by clicking on the 15-Passenger Van Course Materials/Forms link. Note: If your department has/uses its own 15-passenger vans, then use the Departmental Van Use form; this restricts use of 15-passenger vans to in-town use only! For other campus departments, or if you intend to operate a 15-passenger van outside Town of Boone limits, then use the Motor Pool Van Use form.
2. Applicants and sponsoring supervisors must complete the top portion of the form, including the requested information inside the box area (located below item #3 on the form).
3. Send or take the form to University Police to complete the applicant's driving record check. The form will be returned to the sponsoring department's supervisor or applicant if personally delivered, following the driving record check.
4. Applicants must complete the on-line 15-Passenger Van Online Awareness Course AND Test. The online course and test are available on the Safety & WC Office website (www.safety.appstate.edu) by clicking on the 15-Passenger Van Course Materials/Forms link. Copies of the test results are forwarded to the applicant and the Safety Director. Following course/test completion, sponsoring supervisors must forward the 15-Passenger Van Driver Approval Form to the Safety Director for approval (signature). As an option, an applicant can take his/her copy of the 15-Passenger Van Online Awareness Course Test results and the 15-Passenger Van Driver Approval Form to Bob Ellerbe for approval (signature).
5. The sponsoring supervisor must contact Bob Ellerbe, Motor Pool Director, to schedule and complete the driving portion of the course. The Motor Pool Director will sign the form to verify completion of the driving portion of the program, enter the applicant's information into the Motor Pool database, and return the completed form to the sponsoring department's supervisor to satisfy record keeping and retention requirements.
REMINDERS......
1. There are six (6) specific steps involved in the 15-Passenger Van Driver Approval process; refer to the 15-Passenger Van Driver Approval Forms for specific procedures.
2. The 15-Passenger Van Driver Approval Form - Departmental Van Use is to be printed on Warehouse Goldenrod Paper.
3. Existing 15-passenger van drivers are exempt from completing the 15-Passenger Van Driver process; however, you are strongly encouraged to have all identified 15-passenger drivers complete the 15-Passenger Van Online Awareness Course and Test plus receive "hands on" driver training from Motor Pool officials.
4. The University 15-Passenger Van Driver Approval
process also applies to the use of outside, rental vehicles.
5. Sponsoring supervisors and hiring officials are strongly encouraged to allow sufficient time in scheduling and completing the 15-Passenger Van Driver Approval process for prospective drivers.
6. As a courtesy, please contact Bob Ellerbe in a timely fashion if the applicant is unable to keep the scheduled appointment, or an alternative appointment is needed.
7. Sponsoring supervisors are encouraged to choose prospective 15-passenger van drivers with at least 3 years driving experience.
8. Hiring, sponsoring department supervisors must sign the
15-Passenger Van Driver Approval Form as evidence of sponsoring the prospective driver.
9. In selecting prospective 15-passenger van drivers, academic areas should keep in mind that graduate assistants must be approved on a yearly basis.
10. Department officials and area supervisors should retain completed 15-Passenger Van Driver Approval Forms as a record of designated University 15-passengeer van drivers in their areas. Completed forms serve as a record of completed 15-passenger van safety training for designed drivers in your area.
11. University Police officials do not keep copies of reviewed 15-Passenger Van Driver Approval Forms; reviewed forms are returned to the sponsoring department.
12. The University General Counsel's Office has verified that University
15-passenger vans are covered by the State of North Carolina's liability insurance when used for University purposes. For questions regarding insurance coverage on University vehicles, contact ASU Motor Pool at 262-3195.
