SUBJECT: Hepatitis B Vaccination Schedule for ASU Employees – Summer 2009 to Spring 2010

To simplify the Hepatitis B vaccination process for everyone, Student Health Services will administer Hepatitis B vaccine to ASU employees each month on the following dates during Summer 2009, and Fall/Spring 2010:

Thursday, May 28, 2009 Friday, May 29, 2009 Thursday, June 25, 2009 Friday, June 26, 2009 Thursday, August 20, 2009 Friday, August 21, 2009 Thursday, September 24 , 2009 Friday, September 25, 2009 Thursday, December 10, 2009 Friday, December 11, 2009 Thursday, January 28, 2010 Friday, January 29, 2010 Thursday, March 4, 2010 Friday, March 5, 2010 Thursday, May 6, 2010 Friday, May 7 , 2010 Thursday, June 17, 2010 Friday, June 18, 2010		Vaccine will be administered between the hours of   8:00-11:30 a.m. and 12:30-3:30 p.m.   on the dates shown at left.   Employees must wait 20 minutes after receiving the vaccine.

Employees are reminded that:

• No appointments are required. However, it is suggested that, before the first time you go, you call Student Health Services at extension 3100 to ensure that they have received your form.

• Turn in your completed “HBV Vaccination Consent Form” at the end of the Bloodborne Pathogens Training class. If you fail to do so, mail it immediately to WANDA YATES, SAFETY & WC OFFICE, BUS AFF ANNEX; you must have mailed your form Wanda at least 5 days before you plan to show up for the clinic. (You will receive this form during the BBP Training session.)

• The HBV Vaccination Consent Form must have the signature of both the Approved Bloodborne Pathogens Instructor who conducted your class and your Departmental Supervisor having budget code authorization authority.

• An effort will be made to accommodate employees who ABSOLUTELY cannot meet this schedule. Contact the Immunization Clinic Nurse at extension 3100 to arrange for scheduling.

• There is no charge to the employee. Departments pay $35 per injection (each employee generally recieves a total of 3 injections over a 6-month period).
